| The Jackson Soccer Club coaching staff and Board welcomes you and your player(s) to our club. Your player's success and the club's depends on your commitment to your player and the club. Each team will have a team organizational meeting to answer questions and meet the team members.
You will find the instructions and forms (forms are pdf and require Adobe Acrobat Reader® )below to complete the registration process.
- Download and fill-out the Commitment Letter
- Download and fill-out the Player Expectations Conduct Form
- Provide a copy of the Player's Birth Certificate
- Download and fill-out the Medical Release Form and this form must be notarized
- Provide a deposit check of $150.00 ($100.00 for returning players from Fall 2007 season) made payable to JSC (this check is deducted from dues).
Submit the completed Commitment Letter, Player Expectations Conduct Form, notarized Medical Release, Birth Certificate and check to your player's Team Manager at the organizational meeting.
